How Do We Do It
Detailed onsite assessment
We use our proprietary detailed assessment process to understand every aspect of our clients needs and their family’s challenges. Once you have contracted for IKOR®'s service, an RN Patient Advocate will be assigned to your family to begin the IKOR® Approach and complete an Initial Assessment report.
Initial Assessment
We begin with a comprehensive review of client’s current status and needs. An essential part of this assessment explores the current medical and physical care. This part of the assessment becomes the cornerstone of consideration for all other elements. At IKOR® we believe that the best care is one that maximizes the abilities of our client, and functions within their wishes for treatment. The RN Patient Advocates will consider not only what must be addressed immediately, but what will be needed in the future; so that the client’s care becomes adaptable to meet the ever changing needs of age. At the core of The IKOR® Approach are a myriad of nationally recognized screening tools. The findings gained through these tools are used to fine tune the strategic plan presented to you by IKOR® . Medication Analysis
IKOR® understands that one of the principal courses of treatment is medication. Central to this belief is that successful treatment with medication is a fine balance. As an integral part of The IKOR® Approach, the RN Patient Advocates performs an analysis of the interaction of the client’s medications. Unlike the review provided by the local drug store, the IKOR® analysis incorporates all over the counter and prescription medications including herbal remedies. When conflicts are discovered, the IKOR® RN Patient Advocates can interact with the individual’s physicians to reset the fine balance ultimately improving efficiency and safety.
Environmental Assessment
For many clients, remaining independent at home is the primary goal. Aging in Place is an attainable goal for many, and whenever possible, the goal of IKOR® . Often, when Aging in Place is possible, modifications to the environment must take place. These modifications can be as simple as securing small rugs to the complex structural changes such as bathroom redesign. The IKOR® RN Patient Advocate assesses the current living arrangement and its challenges based upon the findings of The IKOR® Approach. If remaining at home is possible, The IKOR® Approach will detail exactly what needs to be done and why. IKOR® will provide cost projections for the project and how those projections compare to any alternatives.
When remaining at home is not an option, IKOR® will present you with specific placement recommendations that meet the level of care, financial demands, geographic desires, and cultural aspects of the case.
You will be given the tools to make the best decision for your situation.
Care recommendations based on stringent quality standards
Whether recommending a new specialist, facility, or home health care provider, IKOR® deals only with those who provide the highest standard of care. IKOR® reviews each provider’s care standards prior to considering them as a possible resource to our clients. The RN Patient Advocates reviews them once again, just prior to recommending them in our strategic plan. In this way IKOR® can assure they continue to meet not only our high standards but the specific needs of the individual client. Because IKOR® is not affiliated with any home care agency or facility, you can feel confident that the recommendations made reflect only objectivity and quality of life concerns.
In addition to these broad range of services IKOR® can coordinate: transportation, meal preparation, and medication dispensing.
On Site Quality Assurance
We assure quality of care through on site unannounced oversight visits. Conducted by an IKOR® RN Patient Advocates, these periodic spot checks of healthcare facilities and assessment of services ensure that your loved one is receiving the highest standards of care regardless of where the care is provided. IKOR® provides on-site quality control monitoring of all services as part of your existing care plan, as well as for all facilities and services IKOR® recommends.
