Patient Advocacy, Routine Bill Pay, Long Term Living Arrangement and Oversight...A comprehensive solution provided to a
family by IKOR® for their aged loved one.
Issues and Challenges
Ms. T was introduced to IKOR® by way of her brother and sister in law. Ms. T's brother Howard and his wife had been acting as her Power of
Attorney for the greater part of her life due to a long standing mental health illness. Howard had, in the month preceding, suffered a heart attack, and his
wife's health was also on the decline. Just before contacting IKOR®, Ms. T had been hospitalized and where she would live after discharge was
in question. Because of these changes in circumstances, the Powers of Attorney were finding it impossible to address their needs as well as the needs of Mrs.
T. so Howard and his wife sought out IKOR® for answers.
IKOR® Approach
IKOR® took on two unique roles for Ms. T. To address Ms. T's ongoing health and living needs, IKOR® assigned an RN Patient
Advocate. This RN Advocate met with the staff of hospital to discuss her care and worked with the discharge planner to identify the best living arrangement.
Ms. T had tried to live at home but unfortunately could not do so successfully. IKOR® identified an assisted living program prepared to meet
her needs, including the ability to assure that Ms. T was taking her medications as prescribed.
To address Ms. T's need for bill pay support, IKOR® assigned a Personal Needs Coordinator. The PNC worked with Howard's wife to establish a
checking account which could be viewed on line by Ms. T's sister in law and with IKOR® check writing authority. All of Ms. T's bills were
forwarded to IKOR® and paid by the PNC on a monthly basis. When unusual financial needs occurred, the PNC would communicate by email or phone
with Howard's wife of advice her of the need and recommend a solution.
Case Outcome
IKOR® through its RN advocate continues to oversee the medical and personal needs of Ms. T through coordinated efforts with her facility and
treatment providers. The IKOR® PNC continues to pay Ms. T's monthly bills and additional requirements when they arise as recommended by the
RN advocate. Whenever they wish and wherever they are, Howard and his wife are kept up to date on IKOR® activities and the status of Ms. T's
care through their secure access to Datikor.
