Steve is a fifty-year old man, who was involved in an accident resulting in a permanent head injury. Steve's father was in
his late 70's, and he was concerned that if something were to happen to him, Steve would be institutionalized. Through assessment process
IKOR® RN Patient Advocate identified that Steve's father is a sole care provider and needs outside caregiver. He was provided a routine
respite from the physical and emotional toll of being the sole care provider. IKOR® also ensured Steve's emotional wellbeing through
observing his acceptance and comfort. IKOR® maintains Steve's medical records.
Issues and Challenges
Steve is a fifty-year old man, who was involved in a one car automobile accident one year before resulting in a permanent head injury. Steve's father was in
his late 70's, and he was concerned that if something were to happen to him, Steve would be institutionalized. Through assessment process
IKOR® RN Patient Advocate identified that Steve's father is a sole care provider and needs outside caregiver. He was provided a routine

IKOR ApproachSM
Steve was assigned an IKOR® RN Patient Advocate who completed an initial assessment thereby understanding Steve's current needs and living
environment. After compiling all of the necessary information, the RN Patient Advocate, created a strategic plan of action. Some information was used
immediately to address safety issues within the home. Steve's father was encouraged to begin the use of an outside caregiver for two hours a week. In doing
so, Steve became acclimated to a new person in his world, without the crisis of the loss of his father. Steve's father also saw an immediate benefit. His
father was provided a routine respite from the physical and emotional toll of being the sole care provider, while at the same time observing Steve acceptance
and comfort in the situation.
Case Outcome
Both Steve and his father carry a laminated IKOR® card with printed instructions to contact IKOR® immediately in case of
emergency. IKOR® maintains Steve's medical records, including the names of all treating physicians, for that moment when
IKOR® will be called upon to implement the pre-agreed plan.
